Reported history, source records, and clinical assessment stay distinct.

Why this exists

Years of visits. One story that keeps getting retold.

Many women with persistent pelvic pain collect scans, blood tests and visit notes from emergency rooms, urgent cares and telehealth, each in a different system. The history lives in pieces, and the person carrying it is tired of explaining.

We are building one place where those pieces sit together, each with its source, so the next conversation starts from the whole picture.

What happens next.

Patients need room to explain what life feels like. Care teams need the original context and a clear handoff. The plan should make sense to both.
